By registering below I agree not to hold UWA Taekwon-Do  and/or its Instructors liable for personal injury, loss of property or accidents arising from my / my child’s / children's participation in any UWA Taekwon-Do activity. I also confer that I / they are physically and mentally capable to undertake the activities run.

 

I understand that I / my children may be photographed at UWA Taekwon-Do and give consent for these photographs to be used in future UWA Taekwon-Do promotions. To the best of my knowledge all information contained on this sheet is correct.

No refunds are given for missed classes.
